Spravato® (esketamine) is an FDA-approved nasal medication used in conjunction with an oral antidepressant for treatment-resistant depression and major depressive disorder with acute suicidal ideation. It works through the NMDA receptor to restore synaptic connections and improve mood.

All sessions take place in-clinic under medical supervision and require a minimum 2-hour observation period following each dose to ensure your safety.

24 Hours Before Your Appointment

  • Arrange transportation: You will not be permitted to drive or operate machinery for the rest of the day after treatment. Arrange a trusted ride to and from the clinic.
  • Avoid alcohol and recreational drugs for at least 24 hours prior.
  • Eat a light meal no later than 2 hours before your session and avoid heavy or greasy foods that may worsen nausea.
  • Limit caffeine intake on the day of treatment.
  • Take routine medications as prescribed unless your clinician instructs otherwise.
  • Notify your provider if you’ve had changes in medical status (illness, infection, blood pressure changes, new medications, or substance use).

Day of Treatment

  • Dress comfortably and bring a light blanket or sweater if desired.
  • Avoid wearing fragrances or lotions near the nose, as they can irritate the nasal passage.
  • Use the restroom before dosing; you’ll need to remain seated for most of the observation period.
  • A clinician will measure your blood pressure and heart rate before administration and periodically throughout the session.
  • You’ll self-administer Spravato® nasal sprays under direct supervision.
  • The session typically lasts about 2 hours, including observation.
  • Common short-term effects include: mild dissociation, drowsiness, dizziness, blurred vision, or feeling “floaty.” These resolve as the medication wears off.
  • Staff will monitor you continuously to ensure your comfort and safety.

Immediately After

  • Do not drive, operate machinery, or make important decisions for the remainder of the day.
  • Expect mild fatigue or lightheadedness for several hours; rest is encouraged.
  • Have a ride home and someone check in if you live alone.
  • Hydrate well and eat a light meal once you feel ready.

First 24 Hours

  • Avoid alcohol, recreational drugs, and sedating medications not cleared by your provider.
  • Resume normal activities the next day unless you feel persisting drowsiness or imbalance.
  • Continue your oral antidepressant and any other prescribed medications as directed.
  • Contact your clinic if you experience severe headache, confusion, prolonged nausea, or unusual mood changes.
  • Initial series: Spravato® is typically administered twice weekly for the first 4 weeks, then weekly to bi-weekly for maintenance.
  • Most patients notice mood improvement within 2–6 sessions.
  • Regular check-ins with your prescriber help track response and adjust your treatment plan.
  • Consistent sleep, hydration, balanced nutrition, and therapy complement optimal results.

WHEN TO CONTACT US IMMEDIATELY

  • Thoughts of self-harm or worsening depression
  • Severe nausea, vomiting, or persistent dizziness
  • Chest pain, shortness of breath, or rapid heartbeat
